By submitting this request, I understand and agree to the following:
- Membership holds must be approved by management.
- Holds require a minimum of 2 weeks and may not exceed 3 months per calendar year.
- Billing will be paused during the approved hold period.
- Membership will automatically resume and billing will restart at the end of the hold period.
- Early return from hold must be requested in writing.
